Reablement Carer
Job Summary:
•
To have passion and commitment for quality care and to the
Dependability ‘Ethos of Care’.
•
To share with other staff in supporting the personal care needs
of service users in a way that respects the dignity of the
individual and promotes independence – Enable first, care
second.
•
To help in the care of the care users’ physical environment by
supporting an enriched, stimulating and engaging home.
•
To assist with Activity Support gathering life story information
encourage opportunity for social activity and engagement during
your intervention.
•
To take delegated responsibilities from your line manager in
respect of the service users in your care.
•
To meet all required mandatory training requirements and other
identified learning opportunities that are required to improve
quality care.
Main Duties and Responsibilities:
•
To work with people in their own home and implement a care
plan towards regaining their physical wellbeing and relearning
the skills necessary for daily living, with intensive support over a
6-8 week period.
•
To promote and ensure high standards of care in accordance
with Care Standards.
•
To maximise independence so that the service user is able to
continue living at home
•
To promote and provide high standards of care and support
taking into account the persons physical, mental social and
cultural needs
•
The focus of care is on enablement and empowerment and
reducing the size of any ongoing care package.
•
Ensure the service user is at the centre of decision making and
involve other family members and stakeholders where
appropriate during their reablement programme.
