Occupational Therapist (Mobility Assessments)
Job Summary:
To provide high quality Occupational Therapy Assessments, and other
relevant support to disabled people.
Main Duties and Responsibilities:
•
To carry out full functional assessments of people with
disabilities in their homes and provide an assessment report and
recommendations in relation to equipment and adaptations.
•
To formulate specifications/ design briefs for Major Adaptations
•
To complete an agreed target number of assessments and follow
ups each week.
•
To complete all forms in relation to the occupational therapy
process e.g. assessment form,
•
equipment order forms, wheelchair referral forms etc.
•
To produce typed reports within specified timeframes
•
To support office based senior Occupational Therapist and
administrators in relation to contract requirements.
•
To manage time and case load effectively.
•
To liase with members of a multi disciplinary team as required
e.g. care managers, representatives, technical officers, and
surveyors.
•
To carry out Mobility assessments of service users applying for
mobility concessions including Taxi-cards, Blue Badges, Disabled
Freedom passes and Disabled Parking Bays in accordance with
procedures and relevant legislation.
•
To read and assess mobility concessions applications to ascertain
eligibility according to agreed procedure and legislation
•
To carry out mobility assessments in a clinic setting at an address
specified by Dependability in a fair and transparent way and
taking into consideration overall functional disability.
•
To write concise reports of clinic assessments and input data on
the computer.
•
To work in the community or within the existing office system.
•
To meet targets for assessments and alert line manager of any
problems/difficulties with meeting of targets.
•
Provide cover for other staff during periods of annual leave,
sickness etc.
•
To attend training as required.
•
To adhere to all Dependability’s policies and procedures.
•
Any other duties appropriate to the post as may be required.
•
To assist with administrative duties in answering enquiries from
clients and care managers in relation to occupational therapy.
•
